Student Services Written Complaints Form

If you have a complaint that you wish to register this is the place to begin. Please fill out the required fields and click "submit". You will receive a written reply from the Dean of Student Services within 5 work days. You might be asked to appear to verbally describe the situation which has generated the need for a written complaint. If you have questions about the written complaints process, please contact the Student Services Office at 606-474-3151, or visit the office in Lusby Center. The Written Complaints Process is detailed in the Student Handbook, page 5.
